How do hormones cause skin blemishes?

Androgens such as testosterone and dihydrotestosterone (DHT) play the main role in the development of hormonal acne. Important: The skin is not just a passive receiver, but partly produces and activates these hormones itself. Androgens can therefore act locally in the skin, even if androgen levels in the blood are completely normal. This is precisely why many women with blemished skin have normal blood values. The local skin environment is crucial.

Androgens stimulate the sebaceous glands to produce more sebum and promote the keratinization of skin cells. Both together clog the pores. In this oily environment, the bacterium Cutibacterium acnes multiplies and triggers inflammation, leading to blackheads, pustules, and inflamed nodules.

Not all hormonal acne means too many hormones. Often, the skin is just more sensitive to them.

Women with polycystic ovary syndrome (PCOS) are particularly susceptible because they often have systemically elevated androgen levels. However, hormonal acne affects far more than just them: even with normal blood values, increased skin sensitivity to androgens can lead to blemishes.

Not all hormonal acne is the same. This classification helps you understand what's going on with you:

  • Cyclical Acne: Blemishes that regularly appear before your period, usually on the chin and jawline. Very common and often harmless.
  • Acne in PCOS: Accompanied by irregular cycles, increased body hair, or hair loss. Systemically elevated androgens are often present here.
  • Post-Pill Acne: After discontinuing hormonal birth control, when the skin is readjusting. Often improves after a few months.
  • Acne in Perimenopause: When estrogen declines and androgens relatively predominate, new blemishes can also appear in your 40s.

What influence does diet have?

Diet is one of the most well-studied lifestyle factors in acne. A systematic review from 2025 with GRADE assessment consistently confirms two associations: a high glycemic load and high milk consumption are associated with more severe acne.

Glycemic Load and Sugar

Foods with a high glycemic index, i.e., sugar and highly processed carbohydrates, cause blood sugar to rise quickly. This increases insulin and insulin-like growth factor IGF-1, which in turn can stimulate androgen production and sebum formation. A diet with a low glycemic load, rich in vegetables, legumes, and whole grains, is described positively in studies.

Dairy Products

Several studies link low-fat milk and high milk consumption in particular to acne. This doesn't mean you have to cut out milk completely. However, a conscious approach and trying plant-based alternatives for a few weeks can be worthwhile if your skin reacts strongly.

Omega-3 and the Fatty Acid Ratio

A Western diet with a lot of omega-6 and little omega-3 is associated with increased inflammatory processes. Omega-3 fatty acids from fatty fish, flaxseed, or walnuts are discussed for their anti-inflammatory properties. The direct evidence for acne here is weaker than for sugar and milk, but the safety profile is good.

  • Low-glycemic diet and less dairy: the best-supported nutritional levers.
  • Topical active ingredients: Salicylic acid, retinoids, azelaic acid, and niacinamide are established building blocks of skincare for blemish-prone skin.
  • Clascoterone 1 percent (Winlevi): a newer topical anti-androgen cream. A 2024 network meta-analysis showed significant improvement over placebo. It directly blocks androgen receptors in the skin and requires a prescription.

Promising, but uncertain

  • Zinc: Studies often find lower zinc levels in acne, and oral zinc shows some effect. However, the evidence is not conclusive and weaker than for prescription active ingredients.
  • Myo-inositol and spearmint tea: affect insulin and androgens in PCOS and thus indirectly the skin; direct acne evidence is still limited.
  • Plant compounds like quercetin, resveratrol, and EGCG: antioxidant and anti-inflammatory in research, but clinical data for acne is still sparse.
  • Probiotics and Omega-3: the gut-skin axis is an exciting field, and Omega-3 is plausible and low-risk, but the evidence is preliminary.

Prescription and effective, but with considerations

For severe hormonal acne, doctors also rely on systemic therapies like spironolactone or combined oral contraceptives. These can be very effective but have side effects and should be managed by a doctor. They are not a first step for a few pimples but a real option when the skin is suffering greatly.

Topical active ingredients: from salicylic acid to azelaic acid

Topically applied active ingredients are the first line of defense: they calm existing breakouts and prevent new ones. Choose fragrance-free products with short ingredient lists and introduce new active ingredients slowly so the skin can adjust.

Salicylic acid and niacinamide

Salicylic acid (BHA) is oil-soluble, penetrates the pores and loosens keratinization, ideal for clogged pores and blackheads. Niacinamide (vitamin B3) regulates sebum production, strengthens the skin barrier, and reduces redness, and is very well tolerated. Both are gentle, everyday building blocks that can be combined well.

Retinoids

Retinoids, i.e., vitamin A derivatives like adapalene or tretinoin, are considered standard for acne: they accelerate cell renewal, dissolve keratinization in the pores, and have anti-inflammatory effects. They can initially irritate, so introduce them slowly and consistently use sun protection. Retinoids are taboo during pregnancy.

Azelaic acid, also during pregnancy

Azelaic acid (usually 15 to 20 percent) has antibacterial, anti-inflammatory, and mild keratolytic effects and also brightens dark post-inflammatory hyperpigmentation. Particularly practical for many women: it is considered safe for use during pregnancy, making it an elegant alternative if retinoids are not an option. In one study, 15% azelaic acid gel was similarly effective to an adapalene gel, with good tolerability.

Benzoyl peroxide, tea tree oil, and bakuchiol

Benzoyl peroxide (BPO) has antibacterial effects against Cutibacterium acnes; even 2.5 percent is as effective as higher concentrations but better tolerated, ideal for spot treatment of individual inflammations. Tea tree oil (about 5 percent, diluted) showed a significant reduction in inflammatory lesions in a placebo-controlled study but works more slowly. Bakuchiol is discussed as a plant-based, gentler alternative to retinol; the data here is still nascent.

When it comes to cleansing: gentle rather than aggressive. Too frequent or too strong exfoliation irritates the skin and can even worsen blemishes.

How long does it take for something to happen?

The skin renews itself over several weeks, and hormonal changes take time. Realistically plan eight to twelve weeks for dietary changes and new skincare before you assess the effect. Quick miracles are unrealistic; patience and consistency pay off.
