Polyphenols are found in berries, green tea, olive oil, and turmeric, and hardly any group of substances is celebrated as much in the wellness sector. But what's really behind them, especially when it comes to hormones, the menstrual cycle, and PCOS? We've reviewed the studies and honestly assessed what the research shows and what remains unclear.

What are polyphenols?

Polyphenols are secondary plant compounds, meaning substances that plants produce to protect themselves from stress, UV light, and predators. Chemically, they are identified by at least one aromatic ring with one or more hydroxyl groups. Over 8,000 different structures have been described to date, from simple phenolic acids to complex tannins.

Relevant to us: They are among the most thoroughly researched bioactive compounds in nutrition and are primarily studied for their antioxidant and anti-inflammatory effects.

The five major groups of polyphenols

Polyphenols are not a single substance but a huge family. Researchers usually divide them into five main classes. This distinction is important because individual representatives have very different effects in the body, which explains why polyphenols cannot be lumped together.

How polyphenols work in the body

The common denominator of almost all polyphenols is their antioxidant effect. Their hydroxyl groups can scavenge free radicals, thereby dampening oxidative stress, a state in which more reactive oxygen compounds accumulate in the body than the body's own protective systems can neutralize. Oxidative stress and low-grade inflammation play a role in many hormonal issues, from insulin resistance to menstrual problems.

Beyond this antioxidant effect, many polyphenols interfere with inflammatory and metabolic signaling pathways. In laboratory and animal studies, for example, they influence inflammatory mediators and the sensitivity of cells to insulin. It is important to honestly categorize here: much of this comes from the lab, and not every mechanism that works in cell culture can be directly transferred to humans.

Bioavailability is the crucial point

Many polyphenols are absorbed only to a small extent in the gut and are quickly broken down again. This means that even if a substance has a strong effect in a test tube, often only a fraction of it reaches the cells in the body. Precisely this low bioavailability is one of the main reasons why impressive laboratory results often significantly diminish in human clinical studies.

Polyphenols, Hormones, and PCOS: What the Studies Show

This is where it gets concrete for many of you. PCOS is the most common hormonal disorder in women of reproductive age, and insulin resistance is central to many cases. Because polyphenols target insulin and inflammation, they have been extensively studied as a complementary treatment.

The most significant work to date is a systematic review with meta-analysis by Balieiro and colleagues from 2024. It summarizes 15 randomized controlled trials involving 916 women with PCOS. The result: Compared to placebo, polyphenols significantly lowered fasting insulin levels, BMI, and LH (luteinizing hormone) levels.

Important for context:The effects were statistically significant, which is promising. However, no clear advantage was found for testosterone in this analysis. Therefore, polyphenols are not a substitute therapy, but at best a complementto established measures such as diet, exercise, and, if necessary, medication.

Overview of the most important individual compounds

Curcumin (from turmeric)

Curcumin is one of the best-studied polyphenols in PCOS. Several meta-analyses consistently show an improvement in metabolic markers. A meta-analysis by Simental-Mendia and colleagues (2022) from five RCTs found a significant reduction in fasting blood sugar, insulin, and the insulin resistance index HOMA-IR. However, curcumin had no clear effect on the hormones themselves, i.e., testosterone and LH, in the meta-analysis by Shen and colleagues (2022). Thus, curcumin appears to act more via metabolism than directly via sex hormones. Furthermore, curcumin has shown anti-inflammatory effects in other studies with women. However, further research is needed here as well. Nevertheless, the current study status was strong enough for us at Hormonic to include it in our Hormonic Basetogether with 16 other micronutrients.

Resveratrol (from red grapes)

Resveratrol is the most prominent representative of the stilbenes. A comprehensive review from 2026, which summarizes several meta-analyses, rates the evidence for a reduction in testosterone and LH in PCOS as having moderate reliability. For blood sugar and insulin, the results are less clear and depend heavily on factors such as the weight of the participants. More on this in our detailed article on Resveratrol in PCOS.

Quercetin (from onions, apples, berries)

For quercetin, there are individual RCTs in women with PCOS. In a randomized, double-blind study of 78 overweight women, 1,000 mg of quercetin daily for 12 weeks reduced, among other things, the hormone resistin and improved markers of sugar metabolism. This is promising, but the current data basis is still smaller than for curcumin.

Green Tea (EGCG) and Soy Isoflavones

Green tea, specifically the catechin EGCG, can slightly improve BMI in PCOS in studies. Soy isoflavones have been studied primarily with regard to testosterone and FSH. For both, there are initial indications, but not enough robust large studies to make generalized promises.

Polyphenols through diet: the best strategy

The most honest and simplest recommendation is: Get polyphenols first and foremost from real foods. A colorful, plant-based diet provides not just one, but hundreds of different polyphenols simultaneously, in a matrix that your body is well-adapted to. Observational studies have for years linked a high intake of polyphenol-rich foods to a lower risk of metabolic and cardiovascular diseases. Disadvantage: It can be difficult to achieve the necessary daily dosage through diet.

  • Green tea and matcha (Catechins, EGCG)
  • Extra virgin olive oil (Hydroxytyrosol, Oleocanthal)
  • Cocoa and dark chocolate from 70 percent (Flavanols)
  • Coffee (Chlorogenic acid)
  • Onions, apples, capers (Quercetin)
  • Flaxseed (Lignans)
  • Turmeric, preferably with some pepper and fat (Curcumin)

What to consider with supplements

High-dose single-substance preparations are different from a polyphenol-rich diet. In studies showing positive effects, high dosages are often used, which, as just mentioned, are difficult to achieve through food. This can be useful, but it raises its own questions:

  • Bioavailability: Many polyphenols are poorly absorbed and should therefore be purchased in high-quality forms. The benefit is proven to different degrees depending on the substance.

Are polyphenols and phytochemicals the same thing?

Polyphenols are a large subgroup of secondary plant compounds. Secondary plant compounds is the umbrella term for many bioactive plant compounds, including carotenoids and glucosinolates in addition to polyphenols. So, all polyphenols are secondary plant compounds, but not vice-versa.

Do polyphenols help with PCOS?

The current best evidence, a meta-analysis of 15 studies with 916 women, shows that polyphenols can moderately lower insulin, BMI, and LH in PCOS. The effect on testosterone is unclear, and another meta-analysis found no benefit for many markers at all. Thus, polyphenols can be a complementary measure but do not replace established treatments.
