In a widely noted review paper, Wright et al. (2024) describe an independent "menopause musculoskeletal syndrome" for the first time, thus clearly associating joint pain with hormonal changes.

What happens in the body?

Estrogen is far more than a reproductive hormone. It acts directly on cartilage, tendons, ligaments, muscles, and bones, as these tissues carry estrogen receptors. When estradiol levels drop, a protective factor that previously contributed in the background is lost.

  • Inflammation: Estrogen dampens pro-inflammatory messengers. When it declines, the propensity for silent inflammation ("inflammaging") increases.
  • Cartilage: Estrogen influences water and collagen balance in cartilage and synovial fluid, which can alter elasticity.
  • Musculature: with declining estrogen, strength and muscle mass decrease, which means joints are less effectively stabilized.

Joint pain in midlife is not just a wear-and-tear problem, but often a hormonal signal that should be taken seriously.

Common causes, ordered by evidence

Not every joint pain in midlife comes from hormonal changes. This is precisely what makes careful classification so important.

Estrogen deficiency and the musculoskeletal syndrome of menopause

The strongest current hypothesis: Declining estrogen levels simultaneously alter cartilage, tendons, muscles, and inflammatory processes. Wright et al. (2024) compile the existing evidence for this, but emphasize that it is a newly described syndrome and large intervention studies are still pending.

Loss of muscle mass (sarcopenia)

With declining estrogen and increasing age, muscle mass decreases. Weaker muscles stabilize joints less effectively, leading to more stress on cartilage and ligaments. This can exacerbate symptoms.

If individual finger joints remain permanently swollen, red, or severely painful, an inflammatory-rheumatic disease such as osteoarthritis of the finger joints or rheumatoid arthritis should be medically ruled out.

Other causes that need clarification

Joint pain during menopause is a diagnosis of exclusion. Osteoarthritis, rheumatoid arthritis, and other inflammatory-rheumatic diseases, thyroid disorders, or vitamin D deficiency can cause the same symptoms and require their own treatment.

  • Osteoarthritis: load-dependent pain, often in the knees, hips, or fingers
  • Rheumatoid arthritis: symmetrical joint swelling, pronounced morning stiffness lasting over 30 minutes
  • Thyroid dysfunction: can trigger muscle and joint pain
  • Vitamin D deficiency: common, easy to test and correct

Pain in the finger joints

Many women notice the first joint complaints precisely in their fingers and hands. There is a reason for this: The small finger joints react particularly sensitively to declining estrogen levels. Estrogen influences the water content in cartilage, the elasticity of tendons, and inflammatory processes. If levels drop during perimenopause and menopause, finger joints can feel stiffer, swollen, or tender to the touch.

Morning stiffness of the finger joints is typical: The fingers feel stiff immediately after waking up and can only be bent loosely after the first movements of the day. If this stiffness resolves within a few minutes, it fits well with the hormonally induced pattern of menopause.

Distinguishing it from other conditions is important. Finger polyarthrosis, i.e., osteoarthritis of the finger joints, tends to cause load-dependent pain and nodular thickenings in the joints. Rheumatoid arthritis often manifests as symmetrical swelling and morning stiffness that lasts significantly longer than 30 minutes. If individual finger joints remain permanently swollen, red, or overheated, or if the symptoms persist stubbornly, you should have them medically checked to specifically rule out osteoarthritis, rheumatism, or a vitamin D deficiency.

Hormone replacement therapy can alleviate menopause-related joint pain in some women by stabilizing estrogen levels. Wright et al. (2024) refer to it as a possible option within the musculoskeletal syndrome. Whether HRT is suitable for you depends on your individual risk profile and should be discussed with a doctor.

Micronutrients as a supportive basis
