Hardly any nutrition topic is as emotionally charged as artificial sweeteners. For some, they are the clever trick to save sugar; for others, a health risk. As a doctor, I see my task as neither downplaying nor instilling panic, but rather categorizing what the research actually shows.

Because one thing upfront: too much sugar is well-documented to be unfavorable for metabolism and hormonal health. Sweeteners are a tool to reduce it. Whether they help or bring their own disadvantages depends on the quantity, the context, and the individual body.

What are artificial sweeteners?

Artificial or non-nutritive sweeteners are compounds that are many times sweeter than sugar but provide almost no calories. The most common include aspartame, acesulfame K, sucralose, saccharin, and cyclamate. To be distinguished from these are sugar alcohols like erythritol and xylitol, as well as plant-based sweeteners like stevia and monk fruit. All sweeteners approved in the EU have been evaluated by authorities such as the EFSA and are considered safe within the defined daily maximum intake.

The advantages: why sweeteners are used at all

The main advantage is obvious: sweeteners provide sweetness with almost no calories and cause little to no increase in blood sugar. This can help in reducing sugar and is particularly interesting for people with diabetes or insulin resistance. Unlike sugar, they also do not cause tooth decay. As a transitional aid away from highly sweetened beverages, they can therefore be quite useful.

The open questions: Gut, appetite and metabolism

The indirect effects are where it gets interesting, and research here is not yet conclusive. Three points are most discussed.

  • Gut microbiome:Individual studies suggest that some sweeteners can alter the composition of gut bacteria and, in some people, blood sugar regulation. The effects vary individually and their health significance has not yet been clarified.
  • Appetite and cravings:There is a hypothesis that very intense sweetness can maintain the desire for sweet foods. The data on this is mixed, and clear evidence is lacking.
  • Cardiovascular and long-term:Large observational studies found associations between very high sweetener consumption and cardiovascular events. Such studies show associations, not proven causation.

It is important to classify: Associations from observational studies are indications, not proof. People who consume a lot of sweeteners often differ in other lifestyle factors as well.

And what about cancer?

In 2023, the cancer research agency IARC classified the sweetener aspartame as possibly carcinogenic (Group 2B). This category describes that a link cannot be ruled out, not that a relevant risk has been proven. The responsible expert committee (JECFA) therefore left the recommended maximum daily intake unchanged. Aspartame is therefore still considered safe for normal consumption.

Natural options like stevia, erythritol, or monk fruit are a possible alternative, but even these are not automatically good without limit. The most sensible approach is to generally reduce the habituation to intense sweetness, regardless of its source.
