Eaton and Sethi (2019), in a review paper, describe how closely estrogen, adipose tissue, and female metabolism are intertwined. Simply put: Stable estrogen levels have a beneficial effect on insulin sensitivity.

Progesterone and testosterone also play a role. Progesterone fluctuates throughout the cycle and can influence appetite and hunger in the second half of the cycle. An excess of androgens, i.e., male hormones, in women is often linked to insulin resistance, as typically seen in PCOS. Metabolism is therefore not an isolated system but reacts to the entire hormonal interplay.

When cells become less responsive to insulin over time, it is called insulin resistance. The pancreas then produces more insulin to keep blood sugar stable. Persistently high insulin levels promote fat storage, especially around the abdomen, and can intensify cravings. Insulin resistance is thus often the common denominator behind very different complaints.

This is particularly evident in PCOS. Insulin resistance plays a central role in the development of the syndrome and affects a large proportion of those affected, both lean and overweight women. But even independently of PCOS, insulin resistance is key to understanding why weight and metabolism change in women.

How You Can Recognize Insulin Resistance

Insulin resistance usually develops insidiously and goes unnoticed for a long time. However, there are clues that should raise an alarm: stubborn belly fat that hardly changes despite dietary adjustments, frequent cravings especially for sweets, a significant energy slump after carbohydrate-rich meals, or dark, velvety skin changes on the neck and armpits. An irregular cycle or pronounced PCOS symptoms can also be related to it.

Only a medical examination can provide certainty. Fasting blood glucose, fasting insulin, and calculated values such as the HOMA-IR index can be used to assess insulin sensitivity. These values are also helpful because they often reveal a change long before anything shows on the scale. If you suspect this, it's worth talking to your doctor.

PCOS and the Young Years

In PCOS, insulin resistance is often central. High insulin levels can stimulate the production of androgens in the ovaries, which in turn affects the cycle and skin. Weight is often harder to maintain or reduce in this state because the underlying hormonal situation works against you, not your willpower.

This is an important point for self-perception: those who struggle with their weight due to PCOS are not fighting a lack of discipline, but a changed metabolic state. Menopause and Perimenopause

With the drop in estrogen, metabolism changes noticeably. The body stores fat more strongly in the abdomen, energy consumption can decrease, and insulin sensitivity often declines. A meta-analysis by Jiang and colleagues (2025) evaluated 17 randomized studies with 5,772 women and observed that hormone therapy can reduce insulin resistance in postmenopausal women without diabetes. Important: Hormone therapy is a medical decision that belongs in the hands of a doctor and is not comparable to dietary supplements.

After Discontinuing the Pill

Hormonal contraception also affects metabolism. After discontinuation, the body needs time for its own cycle and hormonal balance to readjust. Weight changes during this phase are common and usually temporary.

Some pills influence water balance and appetite, which can shift again after discontinuation. A short-term change in weight or water retention in the first few weeks is normal and says little about your long-term metabolism. More important than the scale during this transition is to find a stable routine of exercise, adequate protein, and good sleep again.

Weight often increases in women not because they eat more, but because the underlying hormonal situation has changed.

What Really Helps: Exercise and Muscles

If there is one intervention that consistently performs well in research, it is strength training. Li and colleagues (2021) summarized in a meta-analysis of twelve randomized studies that regular strength training can lower the HOMA-IR value, a measure of insulin resistance. A larger evaluation of 43 randomized studies from 2025 with predominantly female participants confirmed this trend and additionally showed an increase in muscle mass.

The mechanism is plausible: more muscle means more tissue that absorbs glucose from the blood. This improves blood sugar regulation regardless of what the scale shows. Endurance training complements this meaningfully, but building muscle mass is the lever that is particularly well-supported for insulin resistance.

Stress, Sleep, and the Cortisol Factor

One factor is often overlooked in discussions about metabolism: chronic stress. Under constant strain, the body releases more cortisol, a hormone that raises blood sugar and can weaken the effect of insulin. If the stress level remains high long-term, it can worsen insulin sensitivity and promote the storage of abdominal fat.

Closely linked to this is sleep. Even a few nights of too little sleep worsen insulin sensitivity and increase appetite in studies, especially for sugary and high-fat foods. Good sleep and a realistic approach to stress are therefore not wellness extras, but part of metabolic health. Equally important is sufficient protein. It satiates, supports the preservation of muscle mass, and helps reduce cravings. Fiber slows down the absorption of sugar into the blood, thus supporting a more stable blood sugar curve.

What Supplements Can and Cannot Do

For individual active ingredients, the evidence varies in strength. Inositol is the best-studied in the context of female metabolic health. Greff and colleagues (2023) showed in a meta-analysis of randomized studies that inositol can improve HOMA-IR values and fasting insulin levels in women with PCOS. The evidence is classified as moderate, and not every evaluation reaches the same conclusion. Inositol is therefore promising, especially in connection with PCOS, but not a substitute for the basics.

Important to note: No single food or supplement significantly increases your basal metabolic rate. Metabolism cannot be revved up like an engine. What influences it are your muscle mass, your sleep, your protein intake, and stable blood sugar regulation. Supplements can support a good diet, but they do not replace it.

What you can do yourself

The German guideline for obesity (AWMF) describes a multimodal basic therapy consisting of dietary changes, more exercise, and behavioral changes as a foundation. Translated into everyday life, this means:

  • Ensuring sufficient sleep, as sleep deprivation impairs insulin sensitivity.
  • Having patience with your own body, especially during hormonal transition phases.
