As a Research Fellow, scarcely any nutrient comes up as often in fertility forums as Coenzyme Q10, usually with grand promises about egg quality. The study data is indeed interesting, but significantly more nuanced than the advertising suggests. Therefore, I want to honestly sort this out: What can Q10 truly do for women, and where does the evidence end?

Coenzyme Q10, also known as ubiquinone, is a vitamin-like substance found in almost every body cell. Its primary role is in the mitochondria, the powerhouses of the cell, where it is involved in the production of the energy carrier ATP. Additionally, as a fat-soluble antioxidant, it protects cells from oxidative stress.

What Coenzyme Q10 Does in the Body

Coenzyme Q10 has two central functions: it is an indispensable component in the mitochondrial respiratory chain, where energy in the form of ATP is generated from nutrients, and it acts as an antioxidant that protects cell structures from oxidative stress. The body produces Q10 itself, but this endogenous production decreases with age; in the heart muscle, for example, it is about half the value in young adults by around age 80.

Coenzyme Q10, egg cells, and fertility

This is the area where coenzyme Q10 is most discussed for women, and it's worth taking a closer look. Egg cells are particularly rich in mitochondria and are sensitive to oxidative stress and aging, so it makes sense to support them with Q10. A 2024 meta-analysis of six randomized studies involving around 1,500 women observed that Q10 pre-treatment in cases of reduced ovarian reserve during in vitro fertilizationwas associated with a higher clinical pregnancy rate and more retrieved egg cells.

As encouraging as this sounds, an honest classification is necessary: These findings come from specific IVF situations with diminished ovarian reserve, not from the general fertility of healthy women. Q10 does not make egg cells younger and is not a guarantee of pregnancy. The potential benefit is indirect, working through improved mitochondrial energy and reduced oxidative stress. Coenzyme Q10 for PCOS

There is also interesting data regarding PCOS. A meta-analysis summarized about nine studies and observed that coenzyme Q10 improved fasting blood sugar, insulin, and HOMA-IR values, and slightly lowered total testosterone. The effects were moderate and the studies were short and inconsistent, so Q10 is not a PCOS medication, but at most a possible building block. Through the indirect route of improved insulin utilization, it can support metabolism, similar to inositol, which is the most thoroughly researched for PCOS.

Coenzyme Q10 is central to the cell's energy production, which is why it is often associated with energy and reduced fatigue. An analysis of 13 randomized studies indeed observed a reduction in exhaustion, particularly in people with certain illnesses or increased needs. Additionally, studies showed that Q10 lowered markers for oxidative stress and increased antioxidant capacity. These are laboratory values, not guaranteed everyday effects, but they support Q10's role as an antioxidant.

Dosage, Forms, and Safety

Studies typically use 100 to 300 mg per day, sometimes higher in the context of fertility. Because Q10 is fat-soluble, it is best taken with a fatty meal. There are two forms: ubiquinone and the more bioavailable ubiquinol. However, a clear advantage of one form for actual results has not been proven; most positive studies used ubiquinone.

Coenzyme Q10 is considered well-tolerated at up to at least 300 mg per day, with occasional mild gastrointestinal complaints. Important: If you are taking coumarin-type blood thinners, consult your doctor before taking Q10, as an interaction is possible.

How to Get Q10 Through Diet

The body produces coenzyme Q10 itself, and this self-production decreases with age. Fatty fish, meat, nuts, and vegetable oils provide some Q10 through diet, but in small amounts. Therefore, targeted supplementation can be beneficial if there is an increased need, for example, in fertility treatment with reduced ovarian reserve.
