In my consultation room, I often sit across from a woman who brings a single lab sheet, a highlighted value, and great concern. "My estrogen is too low, isn't it?" And often my first question isn't how high the value is, but: On which day of the cycle was it measured? Because that's exactly what determines whether the number means anything at all.
A hormone panel in women is not a single value, but an interplay of many messenger substances that constantly shift throughout the cycle. This overview categorizes the most important values: what they are, what each indicates, when you should measure them in the cycle, and what their limitations are. This way, you enter your next conversation with understanding, not just a single number.
What is a hormone panel for women?
A hormone panel is the targeted measurement of several hormones in the blood that together describe your hormonal situation. No lab measures "the one" hormone, but a pattern: reproductive hormones, androgens, prolactin, and the thyroid gland together form a meaningful picture.
For example: In cases of irregular cycles and symptoms like increased body hair, the doctor doesn't just look at one value, but at the pattern of LH, FSH, testosterone with SHBG, and possibly DHEAS, to differentiate PCOS from other causes. In contrast, when menopause is suspected, a rising FSH level early in the cycle is more indicative than a single estradiol value. The pattern counts, not the individual number.
Such a determination becomes useful when there is a concrete suspicion or question behind it: an irregular or absent cycle, unfulfilled desire to conceive, indications of PCOS, the transition to menopause, or suspicion of a thyroid disorder. A hormone panel is then a tool for orientation that complements, not replaces, your symptoms. The interpretation of values always belongs to a doctor, as the same number can be read completely differently depending on the life stage, cycle day, and symptom profile.
Which hormone values are important, and when should you measure them?
The relevant values can be grouped into three categories: reproductive hormones (FSH, LH, estradiol, progesterone, AMH), androgens (testosterone, free testosterone, SHBG, DHEAS), and prolactin and thyroid values as important co-players. The reproductive hormones tell the story of the cycle: FSH and LH control the ovaries from the pituitary gland, estradiol rises with the maturing egg, and progesterone from the corpus luteum shapes the second half of the cycle. A high basal FSH value or low AMH indicates a declining egg reserve, while an abnormal LH to FSH ratio can be associated with PCOS.
The androgens and their co-players complete the picture: Testosterone and DHEAS indicate androgen status, with DHEAS primarily originating from the adrenal gland and helping to narrow down the source of elevated values. SHBG helps determine how much testosterone is freely available. Finally, prolactin and thyroid values act indirectly but powerfully: elevated prolactin or an out-of-sync thyroid can inhibit ovulation and make periods irregular. Precisely such indirect chains, from a single value through a mechanism to a noticeable cycle change, make a complete hormone panel so valuable.
