Female Hormone Levels: Which Hormone Values Really Matter

In my consultation room, I often sit across from a woman who brings a single lab sheet, a highlighted value, and great concern. "My estrogen is too low, isn't it?" And often my first question isn't how high the value is, but: On which day of the cycle was it measured? Because that's exactly what determines whether the number means anything at all.

A hormone panel in women is not a single value, but an interplay of many messenger substances that constantly shift throughout the cycle. This overview categorizes the most important values: what they are, what each indicates, when you should measure them in the cycle, and what their limitations are. This way, you enter your next conversation with understanding, not just a single number.

What is a hormone panel for women?

A hormone panel is the targeted measurement of several hormones in the blood that together describe your hormonal situation. No lab measures "the one" hormone, but a pattern: reproductive hormones, androgens, prolactin, and the thyroid gland together form a meaningful picture.

For example: In cases of irregular cycles and symptoms like increased body hair, the doctor doesn't just look at one value, but at the pattern of LH, FSH, testosterone with SHBG, and possibly DHEAS, to differentiate PCOS from other causes. In contrast, when menopause is suspected, a rising FSH level early in the cycle is more indicative than a single estradiol value. The pattern counts, not the individual number.

Such a determination becomes useful when there is a concrete suspicion or question behind it: an irregular or absent cycle, unfulfilled desire to conceive, indications of PCOS, the transition to menopause, or suspicion of a thyroid disorder. A hormone panel is then a tool for orientation that complements, not replaces, your symptoms. The interpretation of values always belongs to a doctor, as the same number can be read completely differently depending on the life stage, cycle day, and symptom profile.

Which hormone values are important, and when should you measure them?

The relevant values can be grouped into three categories: reproductive hormones (FSH, LH, estradiol, progesterone, AMH), androgens (testosterone, free testosterone, SHBG, DHEAS), and prolactin and thyroid values as important co-players. The following table provides an overview of when to measure which value and what it indicates.

The reproductive hormones tell the story of the cycle: FSH and LH control the ovaries from the pituitary gland, estradiol rises with the maturing egg, and progesterone from the corpus luteum shapes the second half of the cycle. A high basal FSH value or low AMH indicates a declining egg reserve, while an abnormal LH to FSH ratio can be associated with PCOS.

The androgens and their co-players complete the picture: Testosterone and DHEAS indicate androgen status, with DHEAS primarily originating from the adrenal gland and helping to narrow down the source of elevated values. SHBG helps determine how much testosterone is freely available. Finally, prolactin and thyroid values act indirectly but powerfully: elevated prolactin or an out-of-sync thyroid can inhibit ovulation and make periods irregular. Precisely such indirect chains, from a single value through a mechanism to a noticeable cycle change, make a complete hormone panel so valuable.

When to measure in the cycle? Timing is key

The correct measurement time for a hormone status is almost as important as the value itself. Because reproductive hormones fluctuate greatly throughout the cycle, a value is only comparable if it was taken during the appropriate phase.

Early in the cycle, i.e., on day 2 to 5 after the start of bleeding, you measure the basal values of FSH, LH, and estradiol. In this early follicular phase, the values are low and stable, making this window suitable for assessing the baseline ovarian function. A high basal FSH value, for example, can indicate declining egg reserve or the transition to menopause. You can find more details in the article on FSH values.

Progesteronebelongs in the second half of the cycle. The mid-luteal phase is most informative, which is around 7 days before the expected period. In a classic 28-day cycle, this is around day 21; for longer or shorter cycles, the date shifts accordingly. Therefore, it is better to count backward from the expected bleeding than forward from the last period. A sufficiently high value indicates that ovulation has occurred. Because progesterone is released in pulses, a borderline result is more likely to be repeated than immediately evaluated.

Androgenssuch as testosterone and DHEAS are best determined in the morning when levels are highest and most comparable; for testosterone, the early follicular phase is also preferred. Since a large portion of testosterone is bound to SHBG, the total value alone is not very informative. Only in conjunction with SHBG can the biologically active, free portion be estimated (Rosner et al., 2007).

Prolactinis sensitive to stress, sleep, physical exertion, and even the blood draw itself. Therefore, it is determined in the morning, on an empty stomach, and at rest. AMH, TSH, and DHEAS, however, fluctuate little throughout the cycle and can be measured on any day. AMH is considered a robust marker of ovarian reserve, although more recent studies show that it fluctuates more than long assumed (ASRM, 2020).

Blood, saliva, or urine: what is really informative?

For a classic hormone status, blood is the best-studied and most widely accepted method. Most reference values used by doctors are based on serum determinations from blood.

The blood testmeasures the concentration of a hormone in the serum and is the standard for FSH, LH, estradiol, progesterone, testosterone, SHBG, AMH, prolactin, and thyroid values. It is well-standardized and provides the numbers on which established reference ranges are based.

The saliva testprimarily detects the free, unbound portion of a hormone. A useful application is the cortisol diurnal profile, where several saliva samples throughout the day reflect the natural course, which is difficult to achieve with a single blood draw. For sex hormones, however, saliva measurement is less standardized, and the results cannot be directly translated to blood reference values.

The urine test, often through a 24-hour collection or as dried urine, can reflect hormones and their metabolites over a longer period. This is interesting for specific questions and in research, such as for metabolite ratios, but it does not replace established blood diagnostics in routine practice. In short: For most questions regarding hormone status, blood remains the reference, while saliva and urine have their place for clearly defined special questions.

How to prepare for a hormone test

A few small things determine how reliable your values are. First, note your cycle day, because without this information, FSH, LH, estradiol, and progesterone can hardly be interpreted. Schedule the blood draw for the morning if possible, as many hormones follow a diurnal rhythm, and arrive rested for prolactin, as stress and the journey to the practice can temporarily elevate the value.

Also, tell your doctor which medications or hormones you are taking, especially the pill, as hormonal contraception significantly shifts many values. Such preparations should be discontinued a few days before the measurement after consulting your doctor.

Limitations of a hormone panel: why a single value is rarely enough

A hormone panel provides a snapshot, not an absolute truth. Hormone levels fluctuate from day to day, throughout the menstrual cycle, and across different life stages, which is why a single number on its own says very little.

Only context makes a value interpretable: In which phase of the cycle was it measured? What life stage are you in, for example, perimenopause? What symptoms do you have, and does the pattern of multiple values fit? A low estradiol level on cycle day 3 is completely normal; the same number in the middle of the cycle would not be. This is precisely why a hormone panel is not a substitute for a diagnosis, and certainly not for a self-diagnosis from the internet.

Furthermore, a single abnormal value is rarely cause for panic. Because hormones are released in bursts, fluctuate from cycle to cycle, and can even be influenced by the blood draw itself, a borderline or unexpected result is usually repeated before conclusions are drawn. Only a repeatedly abnormal pattern that matches your symptoms is truly reliable.

Cervix and Cancer Screening: Pap Smear and HPV Test

Not all women's diagnostics revolve around hormones. A separate, equally important strand is cervical cancer screening with the Pap smear and the HPV test. Both are an integral part of gynecological diagnostics, even though they do not measure hormone levels.

In Germany, cervical cancer screening has been regulated as an organized program since 2020. Women aged 20 to 34 are entitled to an annual Pap smear. From age 35, co-testing with a Pap smear and HPV test is offered every three years (G-BA, since 2020). The Pap smear assesses cell changes and is classified into stages from Pap I to Pap V; the HPV test detects human papillomaviruses, which are the most important cause of these changes.
